Please email your completed Client Form to info@southernpsychiatry.com in advance of your appointment.

In order to complete your assessment Dr Schenk will also require reports and/or copies of all clinical documentation relating to previous mental health treatment. This includes your GP record, Psychiatry reports/letters and Psychotherapy reports/letters.
